Job Summary:

Provide direct and indirect nursing care to patients. Supervises both licensed and non-licensed clinical personnel in the clinic. Works under the direct and indirect supervision of the Medical Director, but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible.

Qualifications:
Bachelor of Science in Nursing minimum, Masters in Nursing or related Health Administration preferred. Must possess a current Louisiana license to practice nursing and a valid driver's license. Five years nursing experience with at least two years supervisory experience and one year caring for the frail or elderly. Able to function well in a team environment. Possess good organizational, communication and supervisory skills. Able to effectively handle difficult and unusual interpersonal situations.

Essential Functions:

Assessment: Performs formal RN assessments (Initial and Semi-Annual) of participants and relays information appropriately to the RN Care Manager assigned to the participant. Supports RN Care Manager as needed in crafting a Care Plan for participants.
  • Collects patient data and completes required forms with appropriate responses; identifies patient's overt problems/needs and sets priorities; identifies problems requiring further referral and/or follow-up; interprets and records latest diagnostic results; performs advanced nursing assessment using critical thinking skills.
Evaluation: Records patient care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems.
  • Completes all documentation in the medical record as required.
Coordination of PACE clinical care: Reviews plan of care when seeing participants in clinic, implementing appropriate nursing processes to achieve plan of care from clinical team as well as other disciplines.
  • Understands participant and caregiver concerns and relays to IDT for consideration in developing or revising the plan of care.
  • Coordinates patient care with other health care personnel and multidisciplinary support services, and evaluates patient care measures instituted when seeing participants in the clinic. Communicates participant changes to team members. Evaluates routine health maintenance needs of participants and coordinates via use of standing orders or communication with providers.
  • Provides vaccinations (either directly or through supervision of medical assistants) or other ordered injections or treatments appropriate to nursing care. Directly performs or supervises blood draws of participants in the clinic.
  • Participates in the coordination of a 24-hour care delivery.
  • Coordinates On Call schedule for nursing and providers and participants On Call when needed.
Implementing/Managing Care: Ensures clinical nursing care provided conforms to accepted practice standards; provides correct treatments and procedures and other care as prescribed and according to patient care standards; demonstrates understanding of age-related characteristics and needs of patients served; understands and respects patient rights and utilizes established mechanisms for management of ethical issues in patient care.
  • Ensures prompt scheduling of Provider appointments indicated by protocol including post-fall and post-institutionalization. Explains nursing procedures in appropriate forms. Provides teaching following discharge from ER, hospital, or subacute care.
  • Oversees scheduling of participants for services in the PACE clinic, including any outside clinical services or providers that come to PACE clinic to see participants.
  • Identifies emergency situations and independently initiates advanced therapy. Arranges EMS services when needed and ensures handoff of participant. Performs rapid clinical triage of participants in the Day Center in whom staff identify concerning signs or symptoms.
Leadership: Using nursing process, delegates nursing care to appropriate personnel; integrates cost effective measures into nursing practice.
  • Recognizes department problems and takes responsibility for documenting same; monitors unsafe working conditions; complies with policies addressing safe working conditions; demonstrates active participation in QI processes.
  • Complies with clinical expectations to meet staffing demands based on patient care needs; recognizes inappropriate and/or ineffective patient care management, and resolves issue/problem and completes written reports.
  • Manages the Medical Assistant, providing oversight of clinical responsibilities and ensuring regular assessment of clinical competencies.
  • Serves as member of the PACE GNO Management Team and acts as liaison between administration and direct care staff.
  • Responsible for human resource and operational management of RNs, LPNs, and MAs.
  • Coordinates with Medical Director and Senior Nurse Manager on evaluation and implementation of Infection Control policies and procedures.
Maintaining clinical workplace: Ensures equipment is functional and regularly assessed. Orders needed supplies and equipment for the clinic needed by nursing and providers.
  • Securely maintains medications and ensures appropriate medication disposal.
