Summary : The SCN Services Director will provide a high level of coordination that will help enhance service delivery across the multiple programs participating in the three Social Care Networks. Under the direction of the Division Director, the SCN Services Director will be responsible for ensuring the efficient development, implementation and operation of the SCN Services Department, which will be responsible for the implementation, delivery and reporting of services delivered across three regional Social Care Networks. The Division Director will collaborate with the Deputy Executive Director for Programs and other senior management in strategizing on range of services, delivery methodology, and evaluating the efficacy of those services to address operational and emerging needs. This role will serve as a representative to bridge connections with internal stakeholders and external community partners. In close collaboration with the CCCS Executive Leadership, this role will be responsible for the implementation of overall internal controls, supervisory support, program development and budgets as it relates to the Social Care Networks.
Essential Duties and Responsibilities :
- Oversee, manage and supervise staff across three Social Care Network regions (Manhattan, Bronx, Lower Hudson Valley) maintaining full compliance with policies and procedures.
- Serve as primary point person for the project and represent the agency at internal and external meetings related to the Social Care Networks and 1115 Waiver.
- Manage services to ensure that client needs are met.
- Engage cross-organizational teams to identify key issues, gaps, strategic needs, and priorities, recommend solutions to address needs, and work with teams to implement solutions.
- Provide training and support for staff to improve skills and knowledge of services, resources and programmatic obligations.
- Ensure compliance with contract and funder requirements – including participating in and conducting internal case reviews, reviewing reports from external databases, and monitoring progress on contractual obligations.
- Oversee timely and accurate submission of program reports, client-specific reports, statistics, and internal reports, and documentation as needed.
- Oversee the preparation and submission of claims to three Social Care Networks in collaboration with Finance and the Executive Office.
- Evaluate programmatic needs throughout the Division and provide regular updates to the senior leadership team.
- Review reports, proposals, concept papers, or any program documentation to be considered for approval.
- Other job related tasks and projects as assigned.

Position Type and Expected Hours of Work :
This is a full-time position that will require working with underserved communities in the New York metro area. Days and hours of work are generally 9 : 00am-5 : 00pm Monday - Friday. Position requires a willingness to travel as needed to sites in the Lower Hudson Valley, and to NYC, as needed. The position is hybrid with about half of the time in-person (office or field), and half working from home. Some weekend and evening hours, if needed.

Pay range : $110,000 - 120,000 per year, commensurate with experience.
