SUD Case Manager
Catholic Charities’ Outpatient Mental Health Program is hiring an experienced, communicative and compassionate individual to serve as a therapist for Summit County. The program offers creative treatment and prevention programs that enhance the strengths of Summit County families while treating each individual with professionalism and respect. We are a faith-based organization that provides professional services without regard to a person’s personal belief system and are certified by the Ohio Department of Mental Health and Addiction Services. We provide comprehensive clinical assessments for mental health and/or substance abuse.
Responsibilities:
- Providing Case Management for clients of our integrated substance use and mental health program. You will be part of an innovative team providing direct service to clients, and coordinating with peer recovery supporters and clinicians.
- Ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
- Provide clients direct service, either in the office or in the field.
- Review and monitor services, reassess needs, and initiate services which are responsive to the needs of clients; provide service coordination.
- Assist clients in developing formal and informal community supports.
- Provide advocacy on behalf of clients.
- Prepare and complete all required case recordings, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
- Attend clinical and administrative staff meetings, multi discipline team meetings as required.
- Review treatment plans and progress of cases with supervisor for continued support, advice, and approval.
- Perform other duties as necessary to accomplish objectives.
Requirements:
- Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience.
- Must have the minimum qualifications of a CMS, CDCA, or LCDC.
- Must have competent oral, written and interpersonal communication skills.
- Must be well organized, detail oriented and have the ability to maintain confidentiality.
- Ability to work with diverse client population, community organizations as well as staff at all levels.
- Must be able to form good working and therapeutic relationships.
