General Responsibilities
- Operates as a member of the program team; conducts choices for care assessments in the hospital, as well as nursing facilities, supporting living facilities and community-based settings.
- Completes emergency assessments in the hospital, as well as nursing facilities and community-based settings.
- Completes follow up phone calls with clients who have been assessed in the community to determine the need and desire for additional services.
- Other duties as assigned within the guidelines of this position.
Essential Functions
- Must possess the ability to work on a fast-paced, unpredictable schedule that is dictated by client need, completing assessments at short notice.
- Inform individuals of all care options prior to admission and provide necessary resources for the individuals.
- Following program guidelines, conduct choices for care assessments within specific and required timeframes.
- Determine eligibility for services including state, federal, and local programs.
- Provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs.
- Has the ability to handle more complex cases; works with clients who may have multiple challenges.
- Create and maintain confidential case records. Meet or exceed all program, funding source, and/or accreditation standards related to case documentation.
- Complete all agency, program and other required reporting or documentation in an accurate manner according to required timeframes.
- Maintain a cooperative working relationship with service providers and community organizations to meet client needs.
- Must possess the ability to work independently and as part of the team, collaborating on client cases as case management will be passed to a different program after the completion of the emergency assessments.
- Maintain required certifications, including the completion of any recertification or annual training requirements.
- Coordinate work schedule for adequate program coverage and utilize agency time and resources effectively.

Requirements
- A Bachelor’s degree is required for this position with preferences in the areas of social work, psychology, or related field with a minimum of one (1) year of experience providing case management services. All degrees must come from a regionally accredited education institution which has U.S. Department of Education approval.
- Experience in program specific population is preferred.
- Must demonstrate fundamental communication and problem-solving skills and ability to work on complex cases
- Ability to work independently and to respond quickly and appropriately in times of crisis.
- Ability to relate well to individuals with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
- Must demonstrate the ability to work effectively with co-workers and other community agencies.
- Demonstrated ability to work effectively and constructively with persons from all racial, diverse cultures, family systems, and social backgrounds.
- Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
- Strong verbal and written communications skills.
- Passage of complete background clearance.
