General Responsibilities
- Operate as a member of the Daybreak Center team.
- Conduct comprehensive needs assessments and provide coordination of services to assist the identified population to gain access to and receive needed services.
- Monitor ongoing client services, advocate as needed for assigned client caseload.
- Provide services in an office or field-based setting.
- Act as agency ambassador in communities served.
- Other duties as assigned within the guidelines of this position.
Essential Functions
- Help newly homeless persons and those at imminent risk of homelessness identify and implement strategies that will prevent them from having to actually enter shelter.
- Develop creative housing solutions unique to the individual such as family mediation, roommate agreements, rental agreements, repayment agreements, verbal and/or written agreements between family members, conversations with probation officers, landlords, etc.
- Coordinate services such as local and long distance transportation for clients who have identified an alternative housing location.
- Engages client family members and/or friends to ascertain willingness to have family placed in other housing alternatives.
- Maintain excellent communications among all parties (shelters, other homeless services providers, other partner agencies, and first responders) to operate a high functioning community shelter diversion program.
- Establish if indicated, a written care or treatment plan with the client and family to enable the client to live with maximum possible independence.
- Provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs.
- Create and maintain confidential case records. Meet or exceed all program, funding source, and/or accreditation standards related to case documentation.
- Conduct educational presentations for professional and community groups as assigned.
- Possess the ability to work independently and as part of a team, bringing concerns or challenges regarding complex issues to the attention of direct supervisor.

Requirements
- Bachelor’s degree in social work, psychology, or related field with one (1) year of experience providing case management services is required for this position a high school diploma with five (5) years of experience in a related field may substitute for bachelor’s degree; preferred is a Master’s degree with preference in areas of social work and a minimum of three (3) months of experience. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
- Experience working with low income and people experiencing homelessness.
- Ability to work independently and to respond quickly and appropriately in times of crisis.
- Ability to relate well to individuals with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
- Ability to work complex cases and problem solve.
- Ability to work effectively and constructively persons from all ethnic, racial, and social backgrounds and with co-workers and other community agencies.
- Passage of complete background clearance, Tuberculosis (TB) Test, and any other required testing or clearances.
- A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
