Salary $44,000
Description-
Provide case management services to families and individuals residing in Permanent Supportive Housing.
Complete intakes, develop service plans, link services including, conflict de-escalation, managing relationships with a broad range of community providers, promoting housing retention through teaching life skills, setting up programming for tenants.
HMIS case management software.
Bi-lingual encouraged to apply.
Work independently and eves/on-call hours 2 nights a week and flex schedule.
Requirements
Bachelor’s degree from an accredited college, preferably in Social Work.
Experience with conflict resolution and mediation.
2-years relevant work experience and demonstrated ability to provide effective services to clients required.
Needs to be culturally competent across all populations. Bilingual a plus.
Driver’s license with acceptable driving history.
