Position: Comprehensive Case Manager
Salary: $22.71/Hour
Catholic Charities is seeking a Comprehensive Case Manager in the Homecoming Program. This is a part-time (29 hour) position. This position is responsible for providing adults, seniors, and their caregivers support and social services to avoid re-hospitalization after a discharge from Sutter Health Tracy, CA. This position will coordinate, advocate, and provide services reflecting each client's needs.
Responsibilities include Serving as the primary contact for clients and their caregivers, conducting client intakes, making initial home visits and referrals, following up on medical requests, and processing payments to vendors.
Desired qualifications:
1. Two years working with the elderly population or a Bachelor's Degree in social work, psychology, or related field.
2. 1-2 years of successful office experience, preferably in a social service organization.
3. Demonstrated ability to speak, read, and write Spanish and/or English fluently.
4. Demonstrated competence with Microsoft Word, Excel, and Publisher.
5. Able to work independently and follow instructions.
7. Valid California driver's license, insurance, and own transportation.
8. Must pass Livescans (fingerprinting & criminal background check).
