Case Manager - CTI (Critical Time Intervention) provides housing related supportive services to families residing at The Family Assessment Center (FAC) The Case Manager -CTI provides comprehensive case management services to assist families to move into and maintain permanent housing. Is responsible for conducting detailed needs assessments relating to the health, educational, housing, financial, employment, and social needs of the assigned families; maintaining a comprehensive case plan to address the identified needs; monitoring the families’ progress in the case plan; to ensure families receive the supportive services and prevent families from returning to homelessness. The Case Manager - CTI must demonstrate an active commitment to Catholic Charities Mission and strive to incorporate the four Core Values in all aspects of daily work.
EDUCATIONAL & WORK REQUIREMENTS: Master’s degree in social work or related human services field, and a minimum of two (2) years of counseling or related work experience.
EMPLOYMENT CLEARANCE REQUIREMENTS: TB Clearance, HI State Criminal Check, Driver’s License, Driver’s Abstract, Access to Vehicle with Current Valid Registration, Safety Check, and No-Fault Insurance, Education Verification
COMPENSATION: $51,990.37 - $59,500.73
