The Family Nurse Practitioner will provide primary care to immigrant patients of all ages at the Esperanza Center Health Clinic. Responsibilities include diagnosis and treatment of acute and chronic illness, ordering laboratory and other diagnostic tests, patient referrals and prescribing and managing medication. The position is 20 hours per week.
JOB DUTIES/RESPONSIBILITIES
- Must be bilingual - fluent in Spanish and English.
- Collaborate with medical and program staff and participate in preventative health programs to facilitate the optimal health of clients.
- Consult and collaborate with the Medical Director, Program Director, staff and clinic volunteers.
- Participate in organizing and directing grant-funded preventive health programs to facilitate optimal health, disease detection, prevention, and management of common illnesses.
- Assist the Medical Director and Program Manager in completing grant reports with data, cases and outcomes.
- Maintain up to date patient clinical records (medications, allergies, problem lists).
- Provide guideline based medical care.
- Collaborate with the Esperanza Health team in the development of Clinical Manuals, Policies and Procedures.
- Experience with chronic disease management, acute care, and minor procedures.
- Identify and refer clients to social services to address social determinants of health.
- Basic understanding of primary care clinic processes, regulations, and procedures.
- Ability to communicate medical information effectively in English and Spanish required.
- Serves as a member of Quality Assurance, and Peer Review Committees.
- Communicates and interacts professionally with culturally diverse population.
- Must remain flexible to the needs of clients, staff and volunteers.
- Ability to work flexible schedule as needed, to include some evening hours.
EDUCATION & EXPERIENCE REQUIREMENTS
- Master’s degree in Nursing and a Certified Registered Family Nurse Practitioner (FNP) license in the State of Maryland with a current active unrestricted license to practice.
- Registered with Maryland Controlled Dangerous Substance (CDS) and Drug Enforcement Administration (DEA).
- 2-5 years as a Certified Registered Nurse Practitioner with relevant clinical experience.
- BLS CPR & AED certified.
