Role Responsibilities:
- Screens applicants, evaluates eligibility, provides necessary referrals, and facilitates the application process for the Family Self-Sufficiency Program.
- Assumes primary case management and support counseling responsibility for caseload, conducting psychosocial assessments and individualized service planning to include specific, measurable short- and long-term goals and objectives.
- Facilitates supportive counseling to ensure readiness for change and adherence to the developed service plan and provides crisis counseling as needed.
- Maintains a flexible schedule, including evenings and weekends as needed.
- Meets productivity expectations for client services.
- Documents all service activity and client information in the client record and in other Agency approved database(s), in compliance with program policies and procedures and to ensure adherence to department and funding requirements.
- Maintains statistical and financial information and submits accurate, timely reports.
- Knowledgeable about and links clients to appropriate community resources.
- Advocates on behalf of clients to ensure access to services for which the client is eligible.
- Completes other duties as assigned.
