What are the responsibilities and job description for the Community Care Coordinator position at Catholic Charities of the Archdiocese of Chicago?
Essential Functions* of the position are detailed below and include any physical requirements checked below:
1. Communicates effectively with clients in both English and Spanish.
2. Works independently and consistently exercises discretion and sound judgment in conducting annual assessments of clients' needs using the prescribed client applications.
3. Coordinates the annual building assessment to identify residents who are frail and/or at risk. Assists residents in the development of the annual service plan. Provide referrals to service agencies and service providers to the residents and monitor service delivery.
4. Assist management when necessary to permanent place a resident in a more appropriate setting. Research and maintain a network of service providers and eligibility for service provisions. Maintain a service directory of local resources.
5. Interact with local agencies and service providers in the community. Completes all required documentation in accordance with agency and programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing, or accrediting bodies in the course of performing job expectations.
6. Facilitate educational programming for residents and family members related to health, wellness, safety, financial assistance, residents rights, and all other appropriate information. Maintains positive relationships with residents, vendors, and staff. Handles all clients complaints as well as crisis situations in consultation with his/her supervisory chain.

Compensation: $22.75-$25.75 / hour
