What you Deliver
- Assist case managers by providing direct support to clients, including helping clients access parenting resources, health services, financial assistance, housing support, and other community services. Follow up with clients on their progress, service plan actions, and upcoming appointments.
- Conduct intake assessments, update client files, and ensure that all client data is accurate and up-to-date. Work with clients to resolve basic issues and barriers to service access, referring complex cases to case managers as needed.
- Help maintain case files, prepare case summaries, and ensure that all necessary documentation is collected for case managers. Support with data entry and tracking client outcomes to ensure program compliance with Thriving Families Grant requirements.
- Schedule appointments for clients, send reminders, and coordinate transportation when necessary. Assist with preparing reports and documentation needed for program audits, funding reviews, and internal reporting.
- Provide clients with referrals to community resources, such as parenting classes, mental health services, childcare, prenatal care, and financial assistance. Build and maintain knowledge of available local services and resources that can support expectant and new parents.
- Help organize and participate in community outreach efforts, including events or workshops related to parenting, pregnancy, and family wellness.
- Assist in facilitating group sessions, workshops, or educational activities focused on parenting skills, prenatal care, and family wellness. Serve as a liaison between clients and service providers to ensure smooth coordination of services.
- Encourage client participation in the program’s various initiatives and support groups. Provide emotional support and encouragement to clients, helping them remain engaged in the program.
- Work closely with case managers, supervisors, and other program staff to provide holistic care to clients. Participate in regular team meetings to discuss client progress, challenges, and successes. Provide feedback to case managers and supervisors on client needs or potential service improvements.
The Expertise We're Looking For
- High school diploma or GED
- Two (2) years’ experience in a social services, community outreach, or case support role.
- Valid Texas Driver’s License
- Basic understanding of parenting, pregnancy-related services, and family support resources.
- Strong interpersonal skills, with the ability to build rapport and communicate effectively with clients from diverse backgrounds.
- Excellent organizational skills and attention to detail.
- Ability to work collaboratively in a team setting.
- Proficiency in using Microsoft Office Suite and basic case management or data entry systems.
