Providing client-centered, wrap-around case management, the Case Worker will support pregnant women, single parents, and families in achieving self-sufficiency and healthy family outcomes. This role requires excellent communication, organizational, and interpersonal skills to navigate and connect clients with a wide range of services and resources.
What you Deliver
- Conduct comprehensive intake assessments and develop individualized service plans in collaboration with clients to address their needs and goals. Provide ongoing case management, including regular progress monitoring and goal refinement. Act as a client advocate, coordinating services across healthcare, social services, education, and employment systems.
- Collaborate with the Blessed Beginnings Manager and Director to facilitate life skills, personal finance, and job training sessions (individual and group).
- Coordinate with Family Support Specialists to facilitate client enrollment in individual and group parenting sessions. Facilitate client access to mental health services through the Counseling Program, providing support for pregnancy-related challenges, including prenatal care, childbirth, parenting, and family dynamics.
- Coordinate prenatal and postnatal care for pregnant women, including appointment scheduling, transportation arrangements, and provider communication.
- Collaborate with medical professionals (obstetricians, pediatricians, nurses) to ensure comprehensive and timely healthcare services. Assist clients in navigating health insurance, Medicaid enrollment, and financial assistance programs.
- Refer clients to internal and external programs, including housing, food, childcare, and utility assistance.
- Conduct outreach to community agencies, faith-based organizations, and service providers to expand referral networks.
- Provide information and support to clients regarding legal issues, immigration services, and other complex needs.
- Facilitate life skills, personal finance, parenting, and job training workshops to promote client self-sufficiency. Collaborate with community partners to offer specialized training (resume writing, interview skills, vocational certifications). Guide clients in accessing educational opportunities (GED, vocational training, higher education).
- Conduct home visits to assess living environments, address safety concerns, and support housing challenges. Provide parenting education, infant care demonstrations, and assistance with nursery setup and home safety.
- Establish rapport with clients and their families in their environments to build trust and facilitate personalized service delivery.
- Perform other duties as assigned.
The Expertise We're Looking For
- High school diploma or GED
- Two (2) years of experience in case management, a focus on maternal and child health, pregnancy support preferred.
- Valid Texas Driver’s License
- Demonstrates expertise in community resources, social service systems, and government programs relevant to pregnancy and parenting support.
- Possesses exceptional communication, interpersonal, and organizational skills.
- Proven ability to collaborate effectively with diverse populations within a team-oriented setting.
- Employs a compassionate and nonjudgmental approach when supporting clients facing challenging circumstances.
- Ability to conduct local travel and home visits as required.
- Demonstrates a strong commitment to the mission and values of Catholic Charities, and adherence to Catholic Social Teaching principles.
