What you Deliver
- Oversee and support a team of Rapid Rehousing staff members, ensuring effective and ethical case management services. Conduct regular team meetings, individual supervision sessions, and performance evaluations.
- Provide guidance, mentorship, and training to staff to enhance their case management skills and service delivery. Ensure adherence to program policies, best practices, and agency guidelines in all case management activities.
- Review referrals from the Galveston Coalition, ensuring timely and appropriate client intake into the Rapid Rehousing Program. Monitor eligibility criteria and ensure accurate assessment and placement of clients. Assist staff members in addressing complex cases, crisis situations, and housing barriers.
- Ensure client files are properly maintained in accordance with agency policies and funding requirements. Monitor case documentation, service plans, and progress notes to ensure quality and compliance.
- Track and analyze program data, performance metrics, and outcomes, providing reports as needed. Identify trends, gaps, and areas for improvement, making recommendations for program enhancements.
- Serve as a liaison between Rapid Rehousing Program, the Galveston Coalition, community partners, and funders. Attend coalition meetings, stakeholder discussions, and partnership events to strengthen housing resources.
- Work collaboratively to develop new housing opportunities and advocate for housing stability solutions.
- Identify training needs for case management staff and coordinate or facilitate professional development sessions. Support the implementation of best practices in case management, housing services, and client advocacy. Ensure staff are up to date with compliance regulations, funding requirements, and community resources.
- Performs other duties as assigned.
The Expertise We're Looking For
- Bachelor's degree in social work or related field
- Three (3) years of experience in case management within the homelessness services, housing programs, or social services.
- One (1) year of experience in a supervisory role leading a case management or social services team.
- NSPIRE training (provided by agency- must pass within first 90 days of employment)
- Valid Texas Driver’s License
- Experience conducting outreach and presentations
- Flexible and effective team player, able to adapt to changes and a busy workplace environment
- Knowledge and experience with housing issues and available resources
- Experience and knowledge with the challenges of and resources for those at-risk of homelessness
- Knowledge of homelessness and related issues, and poverty-related issues
- Ability to work with diverse communities
- Excellent organizational skills and an ability to self-motivate
- Excellent written and verbal communication and people skills
- Proficiency in case documentation systems such as HMIS.
- Intermediate knowledge of MS Office: Word, Excel, Outlook and navigating the Intranet
- Ability to work under pressure, address multiple priorities and meet deadlines
- Ability to identify key issues in problem solving and implement effective and creative solutions
