Screening & Intake
- Provides information to those in the community requesting information regarding the admission process and coordinates appointments, as necessary.
- Screens individuals for program eligibility and service needs
- Provides referrals to individuals who cannot be served or served promptly by connecting them to appropriate resources for the provision of interim services.
- Coordinates and facilitates intakes of individuals, including assessment for possible case management needs.
- Collaborates with an interdisciplinary team to identify and coordinate services for individuals with unmet medical needs identified as necessary prior to admission or immediately after.
- Assist consumers with navigating the programs multidisciplinary intake process.
- Meets weekly with individuals served to provide case management services either directly or by referral to include but not limited to the following areas: medical, psychiatric, dental, and independent living skills.
- Collaborates with counselors to ensure that all referrals have been provided as needed under the treatment and/or aftercare plan.
- Provides education, coaching, and counseling to consumers about HIV and Hepatitis C prevention and practicing proper medication management practices, especially regarding medication for Mental Health, HIV and Hepatitis C treatment, and PrEP.
- Maintains accurate and current case record documentation in physical file and agency database that includes status of referrals provided, successful linkages to care, and challenges to linkage to care.
- Facilitates weekly educational therapeutic groups using material from Your Money, Your Goals; Living in Balance; Practical Skills; and other approved topics.
- Submits group progress notes for each group session conducted.
- Facilitates Supplemental Security Income and Social Security Disability Insurance Outreach, Access, and Recovery (SOAR) services in coordination with the interdisciplinary team to eligible individuals.
- Collaborates daily with interdisciplinary team in the development of consumer treatment plans, the provision of information, referral, and confirmed linkage to care for case management needs included in the aftercare and/or treatment plan.
- Provides weekly updates regarding consumer progress, which includes submission of logs of services provided and status of linkages to case management referrals.
- Coordinates supervision for individuals when attending court or other necessary appointments.
- Participates in the implementation of goals and objectives towards consumers preparedness for independent living upon discharge.
- Maintains monthly contact for 6 months with individuals discharged under the SAMHSA project.
- Completes required logs and tracking of services provided to be reported under SAMHSA project.
- Provides guidance to Case Management interns in the coordination of intake and case management services to individuals.
- Ensures that all files are organized and include all required documentation.
- Participates in the Performance Quality Improvement (PQI) meetings.
- Completes appropriate reporting of incident reports in adherence to regulatory agencies and Catholic Charities Policies and Procedures requirements.
- Adheres to all agency Policies and Procedures, program guidelines, funding, licensing regulations, and accreditation standards.
- Adheres to safety protocols per Catholic Charities when conducting job functions.
- Monitors the compliance of safety and health precautions and monitors staff compliance according to all governing bodies’ policies and procedures.
- Participates in all required or requested meetings and training.
- Completes other tasks as requested by supervisor.
- Work is usually sedentary, but may require long periods of standing, walking, and/or bending and some light lifting.
- Ability to travel throughout Miami-Dade and Broward counties for meeting/training locations.
- Bachelor’s degree in social work or related area of study.
- 1 year experience in a related field.
- Bilingual Spanish required.
- Individual with lived experience, with an understanding of and respect for each individual’s unique path to recovery preferred.
