What Will I Do as a Hoarding Case Manager?
In this role, you will:
- Collaborate with Home Coaches and Therapists to develop individualized care plans for clients.
- Conduct client inquiries and in-home assessments to understand clients' physical, social, and financial needs.
- Assist clients in accessing essential services, such as Medicare, food assistance, family care, and disability programs.
- Monitor client progress by evaluating treatment outcomes, advocating for additional resources, and providing personal support during crises.
- Maintain accurate records by reviewing case notes, logging events, and documenting reports.
- Ensure quality service delivery by partnering with program staff and external service providers.
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
- Two years of experience working with older adults, individuals with disabilities, or people with cognitive, physical, or mental health challenges (preferred).
- Valid driver’s license, reliable transportation, and vehicle insurance (liability coverage: $100,000/person and $300,000/accident).
