**Job Description:**
We are currently seeking a dedicated and compassionate Case Manager to provide support and guidance to our outpatient department clients. This role requires excellent communication and interpersonal skills, as well as the ability to work effectively with individuals from diverse backgrounds.
The successful candidate will be responsible for:
- Providing case management and support to OPD clients
- Facilitating PC groups as needed
- Documenting according to regulatory standards
- Carrying a caseload and working collaboratively with the PC Team
- Interfaces with PC psychiatrist and maintains collateral contacts with family members and providers in the community on behalf of clients
