Catholic Charities of the Archdiocese of Newark is one of New Jersey’s largest non-profit social service agencies with programs and facilities serving Bergen, Essex, Hudson, and Union counties.
We are currently seeking a Full Time Case Manager for our St. Lucy’s Emergency Homeless Shelter, located in Jersey City, NJ. St. Lucy's is an 80 bed emergency shelter in Jersey City that ensures safety and builds stability in the lives of homeless men and women.
Job Duties:
- The Case Manager is responsible for the overall responsibility for delivering social services, intake, assessments, admissions, discharge, develop & implement Service Plans, homeless prevention services and related documentation and progress reports.
- Develop and maintain compliance with HUD, County and DCA essential services standards and related policy and procedures.
- Maintain referrals and communication with County Coordinated Assessment Agency.
- In partnership with the family develop a Housing & Service Plan and provide assistant and linkages in obtaining needed services and achieving clients goals.
Job Requirements:
- Bachelor's Degree required (Bachelor’s degree in social work or related field preferred)
- 2-3 years experience working with the homeless population or related experience required
