What are the responsibilities and job description for the Case Worker - Cohoes position at Catholic Charities of the Diocese of Albany?

We are currently hiring a Case Worker, to work in our Housing Office. As the Case Worker, you would be responsible for:
- Assisting the Case Manager to provide services
- Assist with assessments, service planning, progress notes, information and referrals, and skill building activities
- Meet with the Case Manager to review caseload information
- Documents the casework activities of all clients
- Perform and/or review all psychosocial assessments of each new referral to the program
- Organize and facilitate support groups and social and recreational activities
Case Worker Minimum Salary: $19.00 per hour
Case Worker Maximum Salary: $19.00 per hour
Case Worker Schedule: Monday – Friday 12pm-8pm
Qualifications:
- Associates Degree (preferred)
- 1-3 years of experience
- Valid Drivers License
- Experience working with seniors
- Experience working with diverse populations
