As the Elder Care Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
- Receive and respond to all calls for elder care services.
- Conduct in‐home assessments using program‐specific tools to identify clients’ needs, including health, disability, wellness, financial, housing, family, legal, local resources, and advocacy.
- Collaborate with clients to develop individualized service plans outlining clear goals and objectives to support independent living.
- Assist clients directly or through referrals to access needed services, such as in‐home care, legal aid, or community resources.
- Provide short‐term in‐home counseling to address life‐changing events and needs, offering education and advocacy to clients and families about services and systems.
- Conduct regular in‐person or phone visits to ensure client safety and needs are met, acting as a liaison for families as needed
- Document all client interactions, collateral contacts, and service plans in client records and program databases, maintaining strict adherence to HIPAA regulations and agency confidentiality standards

Elder Care Case Manager Schedule: Mon-Fri (25 Hours) Flexible within office hours
Qualifications:
- Bachelor’s Degree in Social Work or related field
- 3-5 years experience working with the senior population
