As the Harm Reduction Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
- Accept participant referrals from Correctional Facilities
- Make initial contact with eligible participants onsite at correctional facilities prior to discharge
- Review assessments and discharge plans provided by Medical Staff, CASAC or other facility staff
- Utilize assessments and discharge planning provided by correctional facility partners to develop a plan of care
- Document updates to electronic records, shared documents, and other formats as required by protocols, policies, and/or procedures
- Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks
- Provide HIV and HCV Screening as necessary
Harm Reduction Case Manager Minimum Salary: $ 21.54
Harm Reduction Case Manager Maximum Salary: $ 21.54
Harm Reduction Case Manager Schedule: Mon – Fri 8:30am – 3:30pm (37.5 Hours)
Qualifications:
- High School Diploma or GED
- 1-3 years of experience working with a diverse population
