As the Licensed Practical Nurse, you will play a pivotal role in individuals' lives, making a significant impact through: Â
- Â Compile monthly/quarterly statistics and submit them to the Health Care Coordinator.
- Be available for after hour consultations regarding medical concerns.
- Enter all medical information from all doctor appointments onto the computer as well as into the CONNEX system.
- Document all medical information in the client's medical chart.
- Send all medical information in to youth’s respective counties in accordance with our current contracts (ie: email to Albany County.)
- When clients return from the appointment, note and schedule any lab tests, x-rays, referrals and follow-up on appointments. Make Residential Manager aware of all appointments to ensure proper staffing and transportation.
- Must keep an inventory of all medical supplies and keep the medical closet organized. Provide proper supervision to ensure the health, welfare and safety of clients.
- Provide transportation on an as needed basis.

Licensed Practical Nurse Minimum Salary: $Â 24.00/hour
Â
Licensed Practical Nurse Maximum Salary: $Â 30.00/hour
Â
Licensed Practical Nurse Schedule: Monday - Friday (35 hours per week)
- On-call shifts: 2 weeks per month

Qualifications: Â
- High School Diploma or GED
- LPN Certification
- Valid Driver's License
- 5 years in a residential setting and working with a diverse population
