This position is responsible for coordinating the social service needs for clients of Catholic Charities walk-in and mobile clinics.
Conduct intake interviews, complete clinical assessments and discharge practices per program policy and procedures. Acquire and maintain pertinent paperwork/records and develop and implement individual service plans. Complete necessary reports, summaries and aftercare plans. Coordinate and arrange social services activities for clients and completion of applications.
Complete required billing and charting after shift. Attend required meetings and training. Other duties as assigned. Become familiar with and develop liaisons with appropriate human services to best advocate and make referrals for outpatient counseling clients.
Decision making, planning and organizational skills. Problem identification, teamwork, time management, and tolerance for stress. Comfortable with individuals who are homeless.
This position will cover casework for both the walk-in and mobile clinics. Requirement to spend time at the walk-in clinic, as well as travel with the mobile clinic.
Bachelor’s degree required. Preferrable in the human service field. Two years’ experience working in the human services field. Experience with individuals who are homeless preferred.
