Social Services Crisis Case Manager – Downtown Pittsburgh, PA

We’re seeking a compassionate, supportive and creative Crisis Case Manager to join our Allegheny County Outreach Team located in Downtown Pittsburgh to provide crisis intervention and prevention that moves individuals out of homelessness.
With your commitment to service to others along with your education and experience in human services you know the importance of building rapport with clients to help them access the services they need. You will establish a partnership to facilitate much-needed relief to members of a vulnerable population, providing case management and helping clients meet important, practical goals.

Crisis Case Manager Performance Objective Include:
- Collaborates with partner agencies to provide direct outreach and engagement to individuals experiencing homelessness and those at risk of experiencing homelessness.
- Facilitates rapport building with clients that utilize the Red Door Ministries located in Downtown Pittsburgh.
- Provides assessments for clients currently in crisis to ensure appropriate internal and external referrals.
- Provides or arranges for needed supportive services including information and referral to other community services to link clients with appropriate resources.
- Documents all interactions with clients, service coordination information, and goal planning activities in a timely, comprehensive, and accurate manner to ensure compliance with program standards.
- Participates in offsite community outreach events.
- Professionally represents Catholic Charities to community partners/agencies, parishes and other religious organizations to coordinate services, provide and enhance communication, and give appropriate referrals for community services.
- Conducts group educational/life skill services to provide a specialized service with appropriate clients.
- Documents all interactions with clients, service coordination information, and goal planning activities in a timely, comprehensive, and accurate manner to ensure compliance with program standards.
- Develops innovative approaches to better serve the needs of clients.
Minimum Qualifications:
- A bachelor's degree in a social services or human services field or related field is required.
- A minimum of one year of experience is required. Internship experience may be applied.
- The proven ability to interact constructively with community agencies and resources is required.
- Excellent problem solving skills are required.
- Excellent written and oral communication skills are required.
- Advanced skills in Microsoft Office products is required.
- The ability to meet with clients at off site locations as needed is required.
- Act 33/34 and FBI clearances are required.
