Wage: $23.32 - $28.62 per hour, depending on experience
Schedule: Monday - Friday, 8 am to 5 pm

Position Summary:
The Case Manager provides case management and other various clinical services under the direction of the Clinical Supervisor, as needed.
Responsibilities:
Provides direct mental health services to adults and children, both in the office and in the community setting.
- Provides case management and therapeutic services, including education, linkage and referrals to individuals who meet access to care criteria
- Facilitates treatment planning and implementation of the treatment plan focusing on client strengths and risk factor
- Completes progress notes and other required documentation within the required timeframes.
- Provides services to achieve the goals identified on the treatment plan
- Services to be provided may include
- Crisis intervention
- Assisting with applying for benefits
- Assisting with activities of daily living
- Psychosocial support
- Service coordination with other providers, including health care professional
- Providing linkage to establish connections with other community resource
- Transportation as necessary
- Accessing community resources
- Meetings with individual and/or family member
- Obtaining housing or assisting with housing retention
Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to other community resources as appropriate
- Assists clients in linking with critical community and therapeutic based services to facilitate and sustain recovery.
- Assists client with transportation issues to and from various appointment
- Assists individuals with individual needs identified on treatment plans
- Participates in ongoing continuous quality improvement to assure the services provided demonstrate best practice standards and meet all contractual requirements
Attends psychiatric or medical appointments with clients as indicated
Perform other duties as assigned
- Follows agency policies regarding confidentiality and other consumer right
- Represents the agency in a professional manner within the community
- Works well with professionals from other agencies
- Participates in supervision and staff meetings, multi-disciplinary team meetings and other meetings as require
- Interprets as needed
- Back-up to Registration and Records Department as needed
- Perform other duties as assigned
Qualifications
Job Requirements:

Physical Requirements:
This position normally requires that physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill.
Non-Physical Requirements:
Education
- Bachelor’s degree in behavioral sciences or social work preferred
- Associate's degree in behavioral sciences or social work with related experience required
Experience
- Two years of experience in direct clinical treatment services preferred
- One year of experience with adults, children, and families
Special Skills
- Knowledge and skills to work with chronically mentally ill population
- Knowledge of community resource
- Knowledge and experience working with older adult
- Strong family and individual clinical case management skill
- Possess and utilize effective verbal and written communication skill
- Ability to be self-motivated, work independently and as a team member
- Ability to represent the agency in a professional manner within the community
- Culturally competent in delivering services to individuals from distinct background
- Ability to use a computer
- Possesses ability to manage data
- Bilingual (English/Spanish) preferred, but not required
Licensure, Registration, Certification
- Must have or seek Agency Affiliated Counselor Registration or Certification
- Valid Washington state driver’s license and minimum required liability insurance for WA State
- Must be deemed insurable as determined by Catholic Charities’ insurance provider
Typical Working Conditions
- Requires the ability to work flexible hours, including evenings and weekend as requested
- Requires extensive travel in the community
- Requires work in clients’ homes and other community location
- Requires the ability to tolerate exposure to tobacco smoke, domestic animals, household pets and other conditions common to a variety of home environments
Employment is conditional upon:
- Being cleared by criminal background check and fingerprinting when required
