TITLE AND CLASSIFICATION:
Coordinated Entry System (CE) Navigator
Non-Exempt (Hourly) 100% Grant Funded

SUMMARY: The Southeast Kansas CES aims to strengthen coordination and referrals to housing services, integrate new healthcare and mental health partners, and increase access to mainstream benefits for CES participants. .
The position will assist in building collaboration between partners to assess and refer households with housing and service providers.

ACCOUNTABILITIES:
The Southeast CES grant aims to strengthen coordination and referrals to housing services, integrate new healthcare and mental health partners, and increase access to mainstream benefits for CES participants.
  • Identifying and engaging households who are unsheltered, have histories of unsheltered homelessness, or possess severe service needs within the Southeast Kansas region.
  • Enrolling identified households into the CES and ensuring their needs are accurately assessed and documented.
  • Coordinating referrals to housing services and introducing additional partners in healthcare, mental health services, and access to mainstream benefits.
  • Implementing the CoC-wide Landlord Recruitment/Engagement Strategy to increase housing opportunities.
  • Managing the prioritization process to ensure that individuals and families with high vulnerabilities and severe service needs are prioritized for housing placements.
  • Hosting and leading Southeast Case Conferencing meetings and participating in Southeast Regional Planning meetings to ensure collaboration among service providers.
  • Assisting the Regional Coordinator in updating the Regional Plan for Southeast Kansa
  • Assisting the Regional Coordinator in updating the Regional Plan for Southeast Kansas in alignment with CoC strategies and goals.
  • Utilizing the CES as a tool for Street Outreach programs and activities.
  • Involving persons with lived experience of homelessness in providing input into the design of best practices, policies, and CES operations.

EDUCATION: Required: Bachelor’s degree in human services or experience in the social work field. Preference to applicants possessing degree in social work, sociology or psychology.

CERTIFICATION, REGISTRATION, OR LICENSE:
Class C driver’s license, personal motor vehicle, and personal motor vehicle insurance required.

EXPERIENCE: Minimum of two years professional experience in housing, employment or social work and success working with low-income, disabled, economically disadvantaged families required. Preference will be given to applicants who have work experience. Minimum of 2 years of experience with data entry, data analysis, or related professional experience.

SKILLS AND ABILITIES: Demonstrated competency in use of personal computer via mouse and keyboard, familiarity with Windows and Microsoft products. Preference will be given for applicants who have Knowledge of homeless issues and concerns.
  • Knowledge of federal policy, program guidelines, documentation practices, and reporting procedures for HMIS and CES is highly preferred.
  • Ability to pass a background check.
  • Ability to establish professional and productive relationships and work cooperatively with individuals, groups, and organizations.
  • Strong written, verbal, and presentation communication skills. Ability to communicate clearly.
  • Strong time management skills and the ability to multi-task and prioritize in a fast-paced environment, while exercising sound judgment.
  • Strong analytical skills and abilities.
  • Proficient in computer operations as well Microsoft Outlook, Word, Excel and PowerPoint.

EXPOSURES: Controlled inside work environment with noise/vibrations from general office equipment. Minimal exposure to fumes/dust/odors.

PHYSICAL DEMANDS: Work Type: Light, exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
Climbing/Balancing/Freedom to Move: May be assigned to multilevel building with some floors accessible by stairs only. Minimal required to move about office. Sit about 85 percent of time.
Stooping/Bending: Minimal.
Reaching: Occasional, overhead as well as horizontal and down.
Vision: Adequate to perform essential functions.
Sitting: Up to two hours without a break (at desk, in a car).
Color vision: No particular color vision requirement.
Hearing: Frequently perceive nature of sounds by ear.
Speech: Frequently express ideas by means of spoken words.
Eye/Hand/Foot Coordination: Frequently operates equipment requiring moderate ability.
Manual Dexterity: Frequently operates equipment requiring moderate ability.
