POSITION DESCRIPTION (Pediatric Interim Care Services (PICS) Home-Based Caseworker is expected to provide:
- In-home support and services to birth families with drug-exposed children who are at risk of out-of-home placement.
The PICS Home-Based Caseworker carries a caseload of 10 to 15 clients and is available by cell phone to clients for help in times of crisis, especially to prevent incidents of child abuse or neglect. Responsibilities include development of a crisis/safety plan with the parent, provision of services and supports to help stabilize the child's level of functioning, and to assist the child's family in acquiring skills and developing supports which will strengthen the parent/ child relationship. The PICS Home-Based Caseworker is committed to the child's right to belong to a family, and committed to preserving, whenever possible, a child's connection to the family of origin.
DUTIES AND RESPONSIBILITIES
Home-Based Case Management:
Provide in-home support, education, and intervention to families with children who are newborns up to age 3 who were exposed to drugs before birth and placed in the care of their parents by Child Protective Services.
- Employee accomplishes this responsibility by completing the following tasks:
- Communicate role, responsibility and agency philosophy in order to assure mutual respect, confidence and trust with the family and other stakeholders.
- Create a professional relationship by providing non-judgmental, support to the family
- Develop a crisis/ safety plan with the family, while maintaining a focus on strengths, needs and creative solutions.
- Completing an intake assessment and assisting parents to identify personal and
family strengths, needs, and achievable goals.
- Visiting the child and parents in the home at least 4 times per month.
- Providing Promoting First Relationships to program fidelity.
- Assisting parents/ family in implementing a workable plan to meet goals, decrease risk, and enhance the possibility of success.
- Monitoring the health and development of the infant/ toddler through routine ASQ3 assessments.
- Referring parent/ child to services and supports in the community to enhance parenting skills, early intervention, and recovery supports.
- Providing in-home parenting education regarding the special needs of drug affected infants.
- Assures that documentation, written and online, is completed in a timely and accurate manner in accordance with contract requirements.
- Completes progress notes on time for each service providing a clear, concise description of the client’s progress towards goals.
- Responsible for upholding a culture of client confidentiality
- Report suspected child abuse to supervisor and/ or CPS as required
- Work interdependently with others toward common goals.
- Negotiate and facilitate transitioning to other services and natural support systems, providing contacts of who will be helpful, and providing follow up.
Case Coordination:
Coordinate case management with assigned DCYF social worker. Employee accomplishes this responsibility by completing the following tasks:
- Consulting regularly with the assigned DCYF/ CPS social worker, communicating progress and concerns regarding parent and child health.
- Attending case staffing relative to the child and parents
- Completing and submitting reports and progress updates timely.
Meetings:
Employee accomplishes this responsibility by completing the following tasks:
- Attend and participate in regular staff meetings.
- Attend and participate in CCSWW meetings as necessary.
- Attend and participate in other related professional meetings as necessary or required.
Professional Development
Participate in available training opportunities; especially as they relate to work with substance abuse and recovery, prenatal drug exposure, child development, and other related issues.
Other duties as assigned.
Minimum Qualifications
- BA or BS in Social Work or related field and two years of relevant experience.
- Good written and oral communication skills.
- Ability to work effectively in high stress situations.
- Good organizational skills.
- Applicant must successfully pass required background checks prior to an offer of employment
- Must have reliable transportation, valid driver's license, automobile insurance and
- Provide current auto registration.
- Must have an acceptable driving record per CCS' driving policy, with no prior DUI convictions (required to provide current driving abstract)

Ability and experience working as part of a team
- Proof of negative TB test within past 12 months and ability or test within first six months of employment.

Preferred Qualifications
- Trained and Certified in Promoting First Relationships
- M.S.W. or MA in Social Sciences or related field.
- At least two years of experience in social services.
- Case management experience.
- Knowledge and experience working with chemically dependent adults and/ or drug expose infants
