AGENCY DESCRIPTION:
Coordinated Care Agency (CCA), a dba of the Archdiocesan Housing Authority, offers comprehensive services, enhanced shelter, and housing for people experiencing deep poverty and homelessness.
The purpose of our work is to ensure that people have access to the healthcare and supports that they need to thrive. The team works to promote social changes to dismantle systemic racism, injustices and inequities that contribute to housing instability. We approach our clients and colleagues through a trauma informed lens, based on the knowledge and understanding of how trauma affects people's lives.
PROGRAM DESCRIPTION:
The Family Connections Team (FCoT) partners with households identified by Coordinated Entry for All (CEA) to connect them with community resources. The primary purpose of these relationships is housing support, including enrollment in Foundational Community Supports (FCS).
The King County Housing Authority (KCHA) – EHV Navigation program provides EHV voucher recipients with supportive housing services with the ultimate goal of obtaining and maintaining housing. Participants are identified through various community partners.
Three pillars guide our team: Housing First, Trauma-Informed Care, and Motivational Interviewing. Using strengths-based approaches, the team moves people into housing options that are realistic, stable, and appropriate as identified by the household. With various partners and stakeholders, the team ensures all services are client-driven.
POSITION DESCRIPTION:
Our mission is to provide a hospitable and low-barrier program with a harm reduction and housing-first model. This is to be done in the spirit of the ministry of presence, which fosters attention to the individual and respect for each person’s unique situation. Our programs are for a population of highly vulnerable people. These people may be high needs, have severe physical disabilities, chronic and acute mental illness, active chemical dependency, and trauma. Our programs emphasize moving people into or maintaining permanent housing through case management. CCA is committed to making our services, our agencies, and our communities free of the divisive and dehumanizing ravages of racism. The Program Manager I is responsible for comprehensively understanding the targeted population of the program, handling specific administrative functions, providing support and oversight of the service management team, and serving as the Program Manager III in the Program Manager III absence. This position incorporates direct client service, advocacy and program administration.
MAJOR DUTIES AND RESPONSIBILITIES:
Supervision:
- In coordination with department leadership, screen, interview, hire, and train staff
- Track and approve expenditures according to contract guidelines
- Meet regularly with each case manager for coaching and supervision
- Coordinate caseloads and distribute incoming clients among the team
- Assess and triage escalated situations
- Foster and maintain positive, working relationships with King County Coordinated Entry for All, Regional Authority, KCHA-FCS partners, and additional partners as identified
- Exercise good judgement and act within program standards when providing financial or other assistance to households
- Ensure staff are screening, enrolling and billing time to Foundational Community Supports (FCS) for eligible clients.
- Regularly review services and client interactions with staff to maximize FCS billing.
- Ensure services are provided in accordance with Medicaid standards for FCS clients.
- Ensure FCS file compliance needs are met for co-enrolled clients.
- Monitor billed FCS Services though out the month to ensure the program is bringing in the target revenue.
- Ensure staff time is billed to the correct contract and cost center.
- Act as the agency liaison with CEA and an extensive network of partner agencies.
- Represent the agency in various community and stakeholder meetings.
- Work with partner agencies and CEA to resolve challenges around client referrals.
Direct Service:
- Ability to travel throughout King County up 5 days per week to meet with participants, various stakeholders, and complete unit inspections
- Attend required meetings/trainings as identified by department leadership and funders
- Employ crisis management skills to respond to households’ immediate housing crisis
- Conduct diversion conversations to explore safe housing alternatives and implement a case plan or referral when appropriate
- Ensure barriers (e.g. translation services, transportation issues, etc.) are effectively addressed
- Assist households to gather supporting documentation required for services
- Have strong time management skills and be able to juggle competing priorities
- Case manage a caseload of 10-12 clients
Data and Standards:
- Input data into the local HMIS and other data entry systems. The HMIS Database is a large, complex database that requires a very high level of attention to detail and comfort working with data systems.
- Ensure client electronic records are kept in accordance with agency and program standards
- Maintain confidentiality best-practices for all interactions with clients, other agencies, and data entry into HMIS.
- Adhere to confidentiality guidelines and respect privacy.
- Ensure participant choice is integrated throughout service delivery.
- Maintain accurate record of hours worked and submit timesheets on schedule.
- Attend required trainings as scheduled.
- Participate in staff and supervisory meetings as required.
- Participate in agency advocacy on behalf of the people we serve

JOB CONDITIONS:
This position also requires the employee to work with clients who may have untreated or poorly treated substance use and mental health disorders. These clients may demonstrate challenging behaviors that require staff support for de-escalation or redirection.
MINIMUM QUALIFICATIONS:
- Ability to gain Agency Affiliated Counseling License or relevant credentials
- At least 2 years of experience in direct provision of social services, preferably with households experiencing homelessness.
- At least 1-year experience supervision in a social service setting.
- Access to private reliable transportation.
- Valid driver’s license and ability to be approved as a CCA Driver
- Excellent written and oral communication skills.
- Experience working with and understanding of data management systems.
- Familiarity with other systems in which participants often participate such as justice, veterans, mental health, foster care, domestic violence, and substance use treatment.
- Understanding of specific barriers immigrant and refugee households face.
- Commitment to working within the mission, goals and objectives of Coordinated Care Agency.
- Criminal history background checks are required prior to employment.
- Fluency in second language that is spoken by a substantial number of King County residents.
