As a Social Worker at CHI St. Vincent Hot Springs, you will join a dynamic team providing care coordination for acute care patients to ensure safe, timely, and appropriate discharge.
You will help care for patients and families dealing with social and mental health issues, chronic diseases, and ethical situations requiring social work expertise to facilitate safe transitions to next-level care.

Responsibilities
Utilize social work expertise to provide care management across the continuum for high-risk patients.
Act as a patient advocate, responding to concerns and working towards resolutions.
Provide health promotion activities and education for patients.
Orient patients and families to community resources, promoting independence.
Address needs affecting patient health, including transportation, financial concerns, end-of-life planning, etc.
Identify home-setting barriers and self-care/management needs, collaborating with patients and caregivers to resolve issues.
Maintain an active caseload of at-risk patients using identification and stratification tools, direct referrals, and other knowledge.
Qualifications
Education: Bachelor of Social Work (BSW) degree required.
Licensure/Certification: Current unencumbered SW license in Arkansas required; Certification in case management (CCM), public health, or community health preferred.
Experience: Three years of social work experience in a healthcare setting, preferably with Medicare and Medicaid populations, and related social/economic issues.
