Job Details :
The Care Manager is responsible for the care planning, utilization of services, coordination of care through weekly conferences, and the education of the teams in the management of patient care services, providing patient centered care efficiently, effectively, and with the best possible outcomes. Implements best practices and evidence based standards of care, moving toward standardization of clinical care for all disciplines. The Care Manager approves / disapproves all patient care plans, new and revised, and acts as the case manager between the teams and insurance companies who require authorization of services.
Posted Salary Range :
USD $58.00 - USD $58.00 / Hr.
