As a Manager of Care Coordination, you will discuss discharge needs with Physicians and Social Workers; and partner with unit nurses to communicate with patients / family about admission and plan of care.
Job Details
Requirements include :
- Active NY Registered Nurse license
- 2-3 years experience in health care related discipline and / or discharge planning or community healthcare experience
- BSN preferred
- Excellent interpersonal skills
- Ability to respond to short deadlines

USD $49.65 - USD $68.10 / Hr.
