Overview
St. Joseph Hospital, located in Bethpage NY, is a not-for-profit full service, 203-bed, community hospital that plays a paramount role in its communities. The Emergency Department cares for over 34,000 patients a year. The hospital has achieved disease specific certification for its Stroke Program which is designated by NYS as a Gold Plus Center. Other vital services include the St. Francis Imaging Center at St. Joseph Hospital, a state-of-the-art cardiac CT scanner, MRI, Vascular Laboratory, Wound Healing & Hyperbaric Medicine, Center for Sleep Medicine, Center for Swallowing, Speech & Voice, and comprehensive in-patient medical, critical care and surgical services.
Responsibilities
The Transitions of Care (TOC) Pharmacist will work collaboratively with healthcare professionals to maximize positive clinical outcomes such as minimizing risk of drug related adverse events, reducing hospital length of stay and reducing readmissions. The Transitions of Care Pharmacist will provide patient-centered care, education, and evidence-based, cost-effective pharmacotherapy. He / she will ensure smooth care transitions. He / she will participate in organizational performance-improvement efforts related to maximizing effective drug therapy while minimizing adverse drug events and hospital readmissions. He / she will serve as a resource to healthcare professionals and participate on institutional committees as appropriate.
- Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.
- Accurately and efficiently evaluate, transcribe and / or verify orders for targeted patient populations.
- Accurately prepare and dispense medications for targeted patient populations.
- Reads, extracts and interprets information in patient charts accurately.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, allergies and appropriateness of drug and dose.
- Discusses drug therapy regimens with prescribers as appropriate.

Requirements :
Education : Graduate of an ACPE-accredited School of Pharmacy with PharmD degree.
Licenses : Current NYS Pharmacist license and registration
Experience : Health system experience preferred, particularly Transitions of Care experience
Pay Range
USD $68.00 - USD $78.00 / Hr.

The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
