HOURS : FULL TIME - MON-FRI 8AM TO 5PM (FLEXIBLE TO WORK SOME WEEKENDS)
- QA CLINICAL EXPERIENCE PREFERREDSummary & ObjectiveThe Licensed Practical Nurse Admissions Supports is responsible for supporting referrals, coordinating patient / family needs for admission to the Hospice Program and coordinating with the Interdisciplinary Team members as required.Essential Functions
- Evaluates assigned referrals for appropriateness for admission and verifies terminal diagnosis and prognosis.
- Review consent forms sent via email or fax with the appropriate person to ensure accuracy.
- Evaluate and insure accuracy in the preparation of documentation.
- Ensure effective communication with Interdisciplinary Teams regarding patient's documentation.
- Enters information provided by the Admissions Nurse and complete the following tasks :
- Face sheet
- PPOC
- Admissions Form
- Clinical Summary
- Order online medical equipment for home admissions
- Provide report to the appropriate Team Manager regarding patient documentation.
- Answers calls from Admissions Coordinators as needed to answer all clinical questions for patients and families.
- Adjusts work assignments and schedule in response to changing patient and / or organizational needs.

- Participates in monthly team meeting at the Main office or by conference call.
- Responsible for the integrity and accuracy of organizational documents and reports.
- Demonstrates knowledge of Catholic Hospice, Inc., policies and procedures and pertinent State and Federal Rules and Regulations.
- Participates in the admission team responsibility of providing seven (7) days week coverage; including evenings and holidays.
- Maintain your required licenses, certifications and mandatory skill updates.
- Comply with all policies, local, state and federal laws and regulations.
- Provide other duties of healthcare team member
- Perform other duties as assignedSupervisory Responsibility

- Associate's in Nursing preferred or Diploma.
- Clear and Active Licensed Practical Nurse license to practice in the state of Florida.
- Current IV Certification required.
- 1-2 years of healthcare or clinical nursing experience preferred.
- Must have knowledge of computer office / clinical software.
- Must be able to read, write and understand the English language.
