Responsibilities:
EEDUCATION
- Master of Social Work (MSW) required
EXPERIENCE
- One year social work experience required
- Two years' social work experience in a healthcare or related setting preferred
- PACE social work experience preferred
- At least one year experience working with the frail elderly
KNOWLEDGE, SKILL AND ABILITY
- Effective communication skills including the ability to assist team members in understanding participant's psycho-social profiles
- Ability to develop and maintain relationships and to work productively with all levels of personnel including physicians, families and care givers
- Strong verbal and written communication skills
