Catholic Health is in search of a Bilingual Licensed Practical Nurse to join our new clinic, Living with Independence for Elders (LIFE) program in Buffalo, NY!
Location: 144 Genesee Street Buffalo, NY 14203
Pay: $22.13 - $33.19/hour based on experience
Shift: 8:30am – 4:30pm, NO WEEKENDS
Summary: Licensed Practical Nurse works under the direction of a Registered Professional Nurse. Performs selected nursing care for participants within the LIFE program. Supports the LIFE clinical team within the clinic, day center and member community. Coordinates medical appointments and provides medical information/forms as needed.
Must Haves:
- New York LPN License
- 2 years of direct patient care (Med/Surg, Certified Home Health Care, LTC, Sub-Acute, Hospice
- Minimum of two (2) years of direct patient care experience in a program for All-Inclusive Care for the Elderly (PACE Program) will be accepted in lieu of above direct patient care experience
- Bilingual (English/Spanish) required
