Catholic Urban Programs (CUP) has partnered with the United Way’s United for Families Initiative to deliver transformative, two-generational case management for families in St. Clair County, Illinois. The Creating New Cycles (CNC) Program addresses intergenerational poverty by providing comprehensive support for both parents and their children, empowering families to overcome barriers and achieve long-term stability.
We are seeking a dynamic change-maker to join our team as a Case Manager. This role is integral to our mission, focusing on personalized, family-centered services that address the unique needs of families while fostering self-sufficiency. The Case Manager will work collaboratively with community partners, utilizing a holistic approach to help families break the cycle of poverty and build pathways to success.
Catholic Urban Programs advances the dignity of the human person, remediates the effects of poverty and empowers people to become self-sufficient, through our crisis intervention, housing security and out of school time programs.
This position is located in East St. Louis IL. The successful candidate will be passionate and enthusiastic about breaking the cycle of generation poverty by equipping some of the most vulnerable and high-risk families with the tools they will need to overcome obstacles and flourish.
Responsibilities:
Case Management:
- Provide 2 Generational Case Management to families experiencing homelessness and those at risk of homelessness.
- Create effective pathways to economic opportunity, including access to mainstream education, training and individualized services for those with barriers to employment.
- Ensure that families have access to economic and social supports to support upward mobility, while also assuring the children’s education has minimal interruptions.
- Help families build social capital that can support both resilience and upward mobility.
- Engage and listen to the voices of the families.
- Complete self-sufficiency assessment and goal setting form
Social Work/Counseling:
- Provide referral to mainstream therapeutic services to ensure families are connected to long-term support to address specialized needs.
- Develop and lead group meetings using creative and out the box thinking, innovative topics to engage families.
- Assist in conflict resolution and behavioral management of children, collaborating with parents/guardians and school as needed.
- Provide crisis intervention counseling as needed
- Attend IEP school meetings (when applicable), supporting and assisting families to self-advocate for their children.
Administrative:
- Attend and fully participate in the monthly United for Families partnership meeting.
- Attend required trainings.
- Facilitate and or coordinate require program workshops, including Financial Asset training.
- Complete monthly programmatic reports
- Submit completed and accurate hardship application.
- Maintain accurate and up-to-date information on client’s progress in the Apricot Database.
- Other duties as assigned and directed by the Executive Director.
Qualifications:
- Master’s in social work or License Professional Counseling Required
- Strong programmatic case management experience.
- Self-Directed and strong initiative.
- Within scope of work, critical thinking skills require decisive judgement and must be able to work independently.
- Outstanding interpersonal skills.
- Intermediate knowledge in Trauma Informed Care practices
- Knowledgeable about social/emotional learning techniques.
- Strong ability to recruit, connect and build authentic relationships
- Experience working in high-risk communities
- Computer literate
- Driver’s license and insurance.
- Clean police record
- Participation in the diocesan child protection training initially and annually
- At least three years of direct social work, counseling and case management experience.
