The Admission Clinical Liaison plays a crucial role in the success of Cavalier Healthcare by serving as a point of contact between the facility and prospective residents, their families, and healthcare professionals. This role is responsible for facilitating the admission process, conducting clinical assessments, and ensuring that all necessary information and documentation are complete and accurate.

Key Responsibilities :
1. Admission Assessment :
- Conduct comprehensive clinical assessments of prospective residents to evaluate their medical needs, functional abilities, and special requirements.
- Collaborate with healthcare professionals, including physicians, nurses, and therapists, to gather relevant information.
- Evaluate the compatibility of the facility’s capabilities with the prospective resident’s care needs.
- Review clinical paperwork and make admission decisions timely.

Any other duties and responsibilities requested by the Owner / Corporate team.
Present with a professional appearance and attitude.
Promote positive relationships with residents, visitors, and government regulators.
Attend and provide in-service as indicated.
Prevent abuse, neglect, and exploitation in the elderly.
Become a participating facility team member with the residents as a priority.
Attend facility meetings as designated by the Administrator.
Qualifications :
Registered Nurse(RN) or Licensed Practical Nurse(LPN) with a valid state license.
Prior experience in healthcare admission, discharge planning, or case management is preferred.
Strong clinical assessment skills and knowledge of relevant medical terminology.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in computer applications and electronic health records (HER) systems such as POINTCLICKCARE.
Understanding Medicare, Medicaid, and private insurance regulations is a plus.
Compassion, empathy, and customer-centric approach to care.
PHYSICAL REQUIREMENTS :

Talking : Communicating with residents, families, physicians, visitors and staff
Hearing : To communicate on the phone effectively, Take instructions from the Administrator, respond to resident complaints and requests, respond appropriately to disaster instructions and pages
Sight : For performing the job effectively and correctly
Smell : For accurate detection and maintenance of facility odors
SUMMARY OF OCCUPATIONAL EXPOSURE :
Tasks and procedures performed by the employee involve risks classified by C.D.C. as Category III. (No contact with blood or other bodily fluids to which universal precautions apply if regular duties are performed.)
OTHER CONSIDERATIONS AND REQUIREMENTS :
Must be able to tolerate a high pace as typical for a nursing facility. Must be able to take and give instructions well and enjoy working with the elderly. Although the employee is required to sit for prolonged periods of time, he / she must also be able to tolerate prolonged standing and walking.
