What are the responsibilities and job description for the Nurse Practitioner position at Cavery Health Clinic?
The Nurse Practitioner provides patient care; conducts physical exams; completes health history summary; documents patient's progress; assists with patient care management with weekly patient visits focused on health, nutrition, exercise and overall wellbeing.
The environment is multifaceted with no “on-call” requirements.
Essential Job Functions
- Obtains and reviews patient's medical and psychosocial history information relevant to patient care
- Conducts physical examinations according to practice guidelines
- Orders and reviews laboratory studies as well as diagnostic procedures; including ECGs. Repeat testing is to be conducted and managed according to the clinic Standard Operating Procedures and as needed and deemed necessary by the practitioner and director.
- Assumes responsibility for prescribing and dispensing medications according to state and federal guidelines.
- Assumes responsibility for the maintenance and accuracy of medication inventory, logs, and reconciliation procedures. This responsibility is considered collaborative with the clinic director.
- Reviews patient's progress and manages patient care accordingly
- Maintains all required documentation for patient care and medication management
- Facilitates referrals to other healthcare professionals and medical facilities as necessary and will communicate with collaborating physician and/ or specialist regarding patient's care
Non-essential Job Functions
- Provides training to clinical and non-clinical staff as needed
- Participates in refresher training as requested by management.
- Documents patient progress notes and treatment plans in the Electronic Medical Record, EMR (eCW).
- Performs other clinical duties as required and needed to complete daily responsibilities and provide patient care
- Conducts follow up calls to patients to discuss the patient's progress and address any concerns
- Provides ongoing support to promote clinic profitability and success
