What are the responsibilities and job description for the Service Facilitator position at Caya Clinic Inc?
CAYA Clinic in Madison, WI is currently seeking a CCS Service Facilitator to join our team of passionate harm reduction professionals.
CAYA Clinic provides affordable treatment options for people who use drugs that would like to use safer, moderate their use, change their relationship with substances, or receive mental health, eating disorder, or trauma therapies with no abstinence requirements.
Mission: To meet people who use drugs in Wisconsin where they are and utilize evidence-based harm reduction therapy techniques to reduce drug-related harms, prevent overdose, and promote the empowerment of people who use drugs. CAYA provides services to all regardless of health insurance status. All participants will have access to harm reduction supplies in addition to therapy and peer services. CAYA has a special emphasis on serving historically marginalized communities through programs created by and staffed by people from their own communities.
Vision: To end overdose and drug-related harms in Wisconsin by creating a community of people who use drugs that are committed to caring for themselves and each other.
Position Summary: The service facilitator provides in community, assessment, recovery planning, individual support, facilitates referrals, and keeps files per DHS 36 requirements. Works with a multidisciplinary treatment team consisting of Clinical Director, Director of Operations, Mental Health Counselors, Substance Use Counselors, and Certified Peer Specialists to meet the clinical needs of each and every client. We are looking for a compassionate, caring, and passionate harm reduction based behavioral health professional. We do offer clinical supervision and education towards receiving your mental health and substance use certification/licensure! This is a hybrid position: some work will be done in community and some can be done in office/virtually.
Principal functions
1. Working with clients to complete intake, multi-dimensional recovery plan, monitors the recovery plan is in alignment with the clients goals, schedules team meetings, places referrals, accompanies clients to medical appointments as needed, and is available for on-going support.
2. Documenting client process through services
3. Coordinating and cooperating with local agencies and organizations necessary to aid treatment for each client and providing referrals for services needed
4. Assist and empower clients to file grievances and complaints
5. Educate clients on current drug trends and steps they can take to keep themselves and each other safe. Provide harm reduction supplies to all clients.
6. Ensuring compliance with local, state, and federal regulations and policies.
7. Performing other tasks assigned by the Program Directors
8. Identifies any clinical/case management needs and works to address those needs.
9. Performs direct one-on-one client contact per week
10. Always acts in the best interests of the client, as the client defines this.
11. Attends supervision as necessary
Minimum Qualifications:
Education/Licensure/Certification: Qualified candidates will have a minimum of a bachelor's degree in social work or related field
Experience Required: Some experience in substance use and/or mental health preferred
Skill and Ability: Necessary to have great interpersonal and communication skills, be able to multitask, prioritize, be dependable and reliable, work in a fast-paced environment, and be able to work well with others. Have a deep commitment to social justice and the empowerment of people who use drugs.
