Cayaba Care is a community-based, maternity program that provides wrap-around support throughout pregnancy and postpartum to improve maternal health outcomes.
Our mission is to improve pregnancy, birth and postpartum journey for women and their families by reimagining how maternity care is delivered. Our team is focused on building trust-based engagement and wrap-around support of expectant moms at home, in-person and virtually to ensure they have the social support they need for a successful and joyful pregnancy.
We ensure that our members do not feel alone during their pregnancy journey, and that they have a trusted resource for support and information throughout their pregnancy, delivery and postpartum.
Cayaba Care does not replace the primary obstetrician or doctor's relationships. Instead, we support expectant mothers with all other support services that are traditionally not available through their doctors' offices. Our multidisciplinary team is led by Maternity Navigators (MNs), who are medical assistants, community health workers, and certified doulas, working closely with our members (in-home and virtually) to ensure they have the care they need to have a joyful and healthy maternal journey.

The Role :
Cayaba Care is seeking full-time Maternity Navigators (MN) who will support Cayaba members through the prenatal and postpartum period. Our Maternity Navigators are the anchor of our work; they are hired from the communities we serve, they understand expectant moms and they build deep and meaningful connections to support our members. The MNs build supportive relationships with our members, meeting with them monthly - virtually or in-person depending on the member's need and preference - to provide key support and resources throughout their pregnancy journey. This role will report to the Director of Care Operations.
Work Schedule : This role is expected to work the following schedule (paid hourly) :
Monday, Tuesday, Wednesday, Friday : 8 : 30am-5pm (hybrid)
Thursday : 10am-6pm (hybrid)
Saturday : 1x a month 9am-1pm (remote)
One week on-call per month
