What are the responsibilities and job description for the Harm Reduction Intensive Case Manager (HOPE Grant) position at Cayuga County Community Health Network?
The position is available as a full-time position (37.5 hours weekly, with a pay of $19.23 per hour, plus benefits) and a part-time position (up to 20 hours weekly, with a pay pf $19.23 per hour). The job provides reimbursement from the office to work-related outings. The main office is in Auburn. This position reports to the Project Director.
Harm Reduction Outreach ICM provides supportive services for a caseload of 15 individuals who are actively using opiates and other drugs. They will also provide referrals to area resources and Narcan Training. Responsibilities include distribution of harm reduction supplies and materials, education of safe use and safer sex practices, referrals to community resources, documenting participant encounters, monitoring harm reduction supplies and ordering supplies when needed.
Position Requirements:
- Proficiency with Office 365 (Outlook, Word, Excel, SharePoint, PowerPoint), Zoom, Facebook, Instagram
- Excellent written and oral communication skills
- Understanding and/or Lived Experience of opiate and other drug use
- Understanding of Harm Reduction theories and practices
- Work collaboratively with other staff
- Ability to work collaboratively with community stakeholders
- Attend trainings and maintain skills
- Clean Driving Record
- Reliable Transportation, Personal vehicle
- Ability to work Independently and in a Team
- Experience with data collection/data analysis
- Lift at least 30 pounds
- Eligible to legally work in the USA
- Other duties as assigned
Experience Requirements
- Bachelor’s degree in Public Health and one-year related experience or Associates degree and two years related experience, or three years’ experience including lived experience (must be at least two years clean).
- Working with opiate and other drug users
- Working in community settings
