Social Worker - Full Time - Days

Join us as a Clinical Social Worker and be at the forefront of addressing the psychosocial needs of those navigating illness, treatment, and transitions into our Skilled Nursing Facility. In this role, you will provide essential support and counseling to residents and their loved ones, helping them cope with the challenges of rehabilitation and placement. Your expertise will be key in ensuring compassionate, personalized care during critical moments of their journey.
Roles and Responsibilities:
- Interview residents and families to assess and identify social, emotional, financial, personal and social psychological, cultural, religious/spiritual needs in order to provide appropriate social work services.
- Communicates with, and coordinates efforts between physicians, nurses, and other personnel regarding resident care plan and/or discharge plan as appropriate.
- Assists patients with financial and material matters when family or friends are unavailable, and provides education and support to residents and families on dementia progression, advanced care planning, and end-of-life decisions.
- Documents psycho-social assessments and services in the resident's medical record in compliance with state and federal regulations, and actively participates in weekly Comprehensive Care Planning and Discharge Planning meetings.
- Maintains liaison with other health care facilities, community agencies, and other appropriate organizations as required in order to build relationships and assist residents with access to community programs and services. Assists residents with access to community programs and services where services are needed Assists Director in the Department's functions, including discussion of Departmental concerns, coverage in the absence of Director, and other duties as assigned.
- Participates in Quality Assurance efforts and serves on facility or organizational committees as assigned.
- Completes Minimum Data Set (MDS) and care planning in line with Centers for Medicaid and Medicare Services (CMS), and NYS DOH regulations.
Required Skills and Experience:
- Bachelor's degree in social work or another human Worker field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology.
Preferred Skills and Experience:
- Three to five years' experience in the healthcare and/or human services field, preferable with the elderly populations and families. Strong written and verbal skills

- Shift: (Mon-Fri) between the hours of 8:00 AM - 6:00 PM
Pay Range Disclosure
- $26.45 to 39.43 per hour
