Hospitalist APP Opportunity located at Schuyler Hospital. Join an Integrated, Innovative, Independent Provider Lead Health Care System with Excellent Quality of Life. This is a full-time, health system employed Hospitalist APP position with a competitive base salary and incentives. We have a full complement of medical and surgical sub-specialty service lines to support our growing healthcare network including ample support from physician, nursing and support staff. Cayuga Medical Center is also home to an Internal Medicine residency program.
We are seeking a 0.8 FTE provider to work approximately nine 10-hour shifts per month in the Med / Surg unit, as well as assume responsibility for covering the SNF on Fridays, with specific hours to be determined based on departmental needs.
Key Responsibilities :
- Discharge patients including summary completion, coordination of follow-up services, home-care orders, & MOLST forms.
- Formulate differential diagnosis, initial diagnosis and initiate treatments.
- Conduct history and physical exams within specified age range.
- Identify medical and health risks and needs.
- Initiate and perform screening and diagnostic procedures within scope of practice.
- Monitor changes in health status and adjust plan of care as appropriate.
- Order and interpret diagnostic / laboratory tests within protocol guidelines and / or in collaboration with collaborating physician.
- Participate in quality assurance review activities.
- Perform Medical Screening Exam (MSE).
- Prescribe pharmacologic and non-pharmacologic agents, devices, or immunization agents under applicable protocols.
- Provide health promotion and maintenance instruction and counseling to individuals, families, and groups.
- Refer and consult with other health professionals, community resources and agencies.
- Write for controlled substances per DEA license and NYS law.
