Cayuga Nursing and Rehabilitation Center is looking for a Case Manager to work alongside the Social Services Director. This position involves responsibility for providing professional social work to residents and their families to aid them in dealing with medically-related social and emotional problems. An employee in this position serves as a member of an inter-disciplinary health care team involving medicine, nursing, dietary, rehabilitation therapies and activities personal in identifying needs and in formulating and implementing the restorative and discharge potential of residents.

TYPICAL CASE MANAGER / SOCIAL WORKER ACTIVITIES : (The following activities are listed as examples only and in no event shall an employee be limited to only those examples listed here.)
CASE MANAGER / SOCIAL WORKER MINIMUM QUALIFICATIONS : Degree in Social Work / Human Service or another related field of study. Prior case management experience required. Past experience working in a Skilled Nursing Facility / long term care.
