Duties
- Assess patient needs and develop comprehensive care plans tailored to individual circumstances.
- Coordinate discharge planning for patients transitioning from hospital to home or other care facilities.
- Collaborate with multidisciplinary teams, including physicians, nurses, and social workers, to ensure seamless patient care.
- Maintain accurate medical records and documentation related to patient care and progress.
- Provide education and resources to patients and their families regarding treatment options and available services.
- Advocate for patients' needs within the healthcare system, ensuring they receive appropriate services and support.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
- Engage with behavioral health specialists when necessary to address mental health concerns impacting patient care.
Experience
- Previous experience in a hospital setting or similar healthcare environment is preferred.
- Familiarity with medical records management and discharge planning processes is essential.
- Experience in hospital medicine or intensive care unit (ICU) settings is a plus.
- Strong background in patient care practices, including knowledge of operating room (OR) protocols when applicable.
- Understanding of behavioral health issues as they relate to overall patient wellness is beneficial.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
