Responsibilities
Manages referral, admission and discharge processes for program
Develops individual Recovery Plans(IRPs) with consumers and completes additional required documentation
Supports and assists consumers with case management duties and crisis intervention as needed
Engages family involvement and act as a liaison between the program and other providers such as local hospitals
Facilitates consumer appointments with APN, court system or outsider providers as needed
Participates in 24-hour Clinical On-call rotation to provide support to staff after hours
Provides orientation training to new employees and ongoing clinical support and direction to direct care staff
Qualifications
MSW in Social Work, MA in Psychology, or a related field
Previous experience in mental health / human services
LSW or LAC preferred
Valid New Jersey driver's license
