* Provide expert nursing care to patients in their homes, as prescribed by physicians or authorized by state/program regulations
* Develop and revise individualized plans of care, working closely with patients, families, and healthcare teams to ensure seamless care coordination
* Communicate effectively with patients, families, and healthcare teams to ensure accurate record-keeping and timely issue resolution
* Participate in quality improvement initiatives and contribute to a positive, supportive work environment
Requirements
* Current RN licensure in state of practice
* One year of clinical experience as an RN, with consideration given to applicants with recent LPN experience
* Excellent communication and interpersonal skills, with ability to work collaboratively as part of a healthcare team
