Job Description
The Registered Nurse in Home and Community Based Services is responsible for providing and directing provisions of nursing and personal care to patients in their homes, as prescribed by physicians or authorized by state/program regulations. You will coordinate the plan of care with other healthcare professionals involved in patient care and help achieve continuity of patient care through effective communication and collaboration.
Key Responsibilities:
- Provides high-quality clinical services within the scope of practice and infection control standards, following the plan of care/service plan and coordinating with other members of the patient/client's care team.
- Evaluates patient eligibility and medical necessity for services, ensuring compliance with payer source and agency policy requirements.
- Develops and revises individualized plans of care and/or service plans according to federal and/or state program requirements, collaborating with other community providers.
- Ensures plan of care frequency and duration meets patient needs, initiating revisions as needed with physician approval.
Requirements and Qualifications:
- A minimum of one year experience as an RN is preferred.
- Current RN licensure in the state of practice is required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are necessary.
- Current CPR certification is required.
