We are seeking a skilled Registered Nurse (RN) to join our team in providing home visits to patients in the Harrodsburg, KY area. Responsibilities
* Provide high-quality clinical services to patients in their homes, as prescribed by physicians or authorized by state/program regulations
* Develop and implement individualized plans of care, in accordance with federal and/or state program requirements
* Ensure continuity of patient care by coordinating with other healthcare professionals involved in care
* Maintain accurate records and communicate effectively with patients, families, and healthcare teams
Requirements
* Current RN licensure in state of practice
* Minimum one year experience as an RN, with recent clinical experience as an LPN considered for waiver in certain cases
* Ability to work independently and collaboratively as part of a healthcare team
