What are the responsibilities and job description for the MDS Nurse - LVN/ RN position at CC Young?
CC Young, a Continuing Care Retirement Community and a not-for-profit leader in Senior Housing and Health Care, has an immediate opening for a MDS Nurse to be responsible for the timely completion and accuracy of the MDS (Minimum Data Set) and care plans to maximize the level of service delivered to the residents.
Shift: Monday through Friday 8:00 am – 5:00 pm.
On call rotation occurs every 9 to 12 weeks
The job duties for this position include, but are not limited to:
- Guarantees the timely and accurate completion of resident assessments and the development of progressive care plans for skilled care residents.
- Maintains the facility assessment calendar, coordinates the completion of all skilled assessments, care plans, and CAAs within State and Federal regulations, and leads the care team in the completion of the Minimum Data Set.
- Checks and resolves Medicare MDS errors.
- Serves as the facility co-liaison with contract therapy provider about Medicare A assessment information.
- Maintains facility records of Medicare use, maintains facility resource manuals for skilled assessments, and ensures the timely completion and submittal of assessments, care plans, and CAAs.
- May perform RN duties as needed.
- Performs other duties as assigned.
Our new team member will possess the following minimum qualifications:
- Possession of a valid license as a registered nurse (RN) or vocational nurse (LVN)
- Previous experience as a MDS Nurse/ Coordinator is HIGHLY preferred.
- Proven ability to prepare, monitor, document and revise Care Plans as needed for Skilled Nursing Residents.
- Proven Knowledge of general, rehabilitative, and restorative nursing and medical practices, procedures, laws, and regulations/guidelines governing Skilled Nursing care.
- Excellent verbal, written and interpersonal skills for interaction with all levels of Company, Resident, Visitor and Vendor personnel.
- Proven computer skills for performing extensive patient documentation/charting.
